Social Worker at Ljusdal-Ramsjö Health Center

Ljusdal-Ramsjö Health Center has approximately 10,500 registered patients. We provide standard primary care services including medical consultations, child healthcare, a psychosocial team, occupational and physiotherapy, day rehabilitation, pool therapy, and dietitian services. We also have a 24/7 family doctor clinic and, in collaboration with the municipality, a nursing department.

JOB RESPONSIBILITIES

As a social worker with us, you will be part of an overarching psychosocial team where we work to increase accessibility for our patients, enabling closer teamwork between our health centers. The role involves close collaboration with other professionals at the health center, and we strive to create smaller teams across professional boundaries to enhance our ability to see the whole picture around the patient and meet their individual needs. We work together from the patient perspective with the vision of "Our shared responsibility for our shared patient," so that everyone we meet feels safe and receives high-quality care.

We are responsible for providing specific short-term treatment interventions within the general medical competence area for patients aged 18 and older, for example, in cases of crisis reactions, depression, anxiety, stress-related illnesses, and psychosomatic conditions. Furthermore, we work to support patients in utilizing their own abilities to improve their life situation and achieve better health. The care pathway work and decision support for stepwise care within primary care and adult psychiatry form the basis for this work.

QUALIFICATIONS

The role requires that you can work independently as well as in a team, and that you are attentive, stable, and show the willingness and ability to help others. You have the ability to plan, organize, and prioritize your work effectively and can make your own decisions and independently drive your work forward. We see you as a person who is responsive and has an easy time collaborating and communicating with others. You contribute to good cooperation through strong engagement, good communication, a positive attitude, and sharing your knowledge.

In the role of social worker, you should:

  • be a qualified social worker or have another equivalent education in healthcare.

For this position, it is advantageous if you have step 1 training, and it is desirable if you are a licensed healthcare social worker. Previous experience in primary care as well as CBT competence equivalent to step 2 is also meritorious. A driving license (B) is considered an advantage for the position.
